I grant Sanitas Management AG (hereinafter “Sanitas”) free and non-exclusive right of use for the image and feedback that I have submitted. This applies for a period of two years from 2022 to 2024.
The content (image and feedback) may be used for the Sanitas website and for internal (intranet) and external (e.g. newsletters, customer portal & apps, social media, campaigns, customer magazine) Sanitas communication, particularly for marketing purposes.
I agree that my personal details may be used by Sanitas to the same extent as the image and feedback that I have submitted. This applies to the image caption with my first name and shortened surname.
Sanitas reserves the right not to publish all the feedback and images submitted.
The declaration of consent applies only to the above communication for the stated purpose and can be withdrawn at any time with effect for the future. In particular, I acknowledge that consent for the use of my image and feedback cannot be withdrawn if Sanitas has already communicated the information.
